You are likely not a good candidate for ALIF surgery if you have severe osteoporosis, have had multiple abdominal surgeries in the past, have one or more vascular conditions, or are not in generally good health. Extreme instability of the spine can also make an ALIF not a good option.

Q: How long does an ALIF procedure take?

ALIF surgeries typically take 2 hours to perform. The length of the surgery can depend on your unique anatomy.

Q: How long will I stay in the hospital after ALIF surgery?

Most patients need to stay in the hospital for 1-3 days after their ALIF surgery. During this time, we will monitor you for any complications and get you up and moving.

Q: What is the recovery timeline after anterior lumbar interbody fusion?

You can expect to have postoperative pain after pain surgery. This typically dissipates within 2 weeks or so. You will be given medications for pain from the hospital. You are encouraged to walk for the first 3 months after surgery. After three months of recovery, outpatient physical therapy can be started. Most of our patients return to light activities and a desk job within a few weeks, but a full recovery can take up to a year.

Q: When can I return to work after ALIF surgery?

Depending on the complexity of your surgery, you will likely be able to return to work in 2 to 6 weeks. It can depend on the type of job you have and how physically demanding it is.

As with any procedure, ALIF does come with some potential risks or complications, including bleeding, blood clots, infection, blood vessel damage, and nerve damage.

Who is a Candidate for ALIF?

A candidate for the minimally-invasive anterior lumbar interbody fusion (ALIF) is typically a patient experiencing chronic, disabling back pain originating from lumbar spine conditions such as disc degeneration, spondylolisthesis, degenerative scoliosis, recurrent herniated disc, and spinal deformity in the lower lumbar segments of the spine. Ideal candidates would benefit from maximum restoration of disc height to relieve pain and have not responded to non-surgical treatments. The procedure is most suitable for those with degenerative disc disease, spinal stenosis, or foraminal stenosis. Patients should have nerve impingement or stenosis resulting in pain, numbness, weakness, or other neurologic dysfunction that significantly impacts their daily activities.

The Anterior Lumbar Interbody Fusion Procedure

Anterior Lumbar Interbody Fusion (ALIF) is performed under general anesthesia, with the patient lying on their back. An expert SpineCare of NY surgeon makes an incision in the abdomen to access the spine, carefully moving aside abdominal muscles, organs, and major blood vessels to reach the affected vertebrae. The damaged intervertebral disc is removed, and bone graft material and a synthetic spacer is placed in the empty disc space to restore disc height and promote fusion. Instrumentation with screws or plates may be used to secure the graft and spacer, and provide additional stability. The incision is then closed, and the patient is monitored as they begin the recovery process. Initial recovery includes managing pain with medication and wearing a back brace for support. In-home physical therapy can start right after surgery, but outpatient physical therapy will start 3 months after surgery, once the bone graft has had time to grow or mature. Most patients can return to light impact activities and deskwork within a few weeks, but full recovery, including the fusion of the vertebrae, can take between six months to a year.
