What is Sciatica?
Sciatica is a type of radiculopathy (pinched nerve) that affects the S1 nerve roots, stemming from the L5-S1 level of the lumbar spine (low back). This condition involves inflammation of the nerve root in the lower spine called the sciatic nerve. The sciatic nerve is the longest nerve in the entire body, starting in the lower back and running down the length of each leg. The pinched nerve can cause lower back pain that radiates down each leg and can also be felt in the buttocks.
What Causes Sciatica?
Sciatica is caused by inflammation that places pressure on the sciatic nerve or damage to the nerve itself. A herniated or bulging disc can lead to sciatica when the disc presses up against the sciatic nerve. Bone spurs can also interfere with the sciatic nerve by poking or pressing against it. Certain medical conditions, like piriformis syndrome, when the piriformis muscle in the buttocks irritates the sciatic nerve or tumors, can lead to sciatica as well. Lastly, injuries of fractures to the lumbar region of the spine can cause sciatica.
Symptoms of Sciatica
Common symptoms of Sciatica include:
- Lower back pain
- Shooting pain down the legs
- Buttocks pain
- Tingling
- Pins and needles sensation
- Numbness
- Muscle weakness
- Urinary or fecal incontinence
How Is Sciatica Diagnosed?
Sciatica is diagnosed through a combination of medical history review, physical examination, and imaging tests. During the physical exam, a physician at SpineCare of NY will assess muscle strength, reflexes, and the ability to perform certain movements to identify the source of pain. They may also perform a straight leg raise test to determine if the pain radiates down the leg. Imaging tests such as X-rays, MRI, or CT scans can provide detailed images of the spine and reveal any herniated discs, bone spurs, or other conditions that may be compressing the sciatic nerve. Electromyography (EMG) may also be used to evaluate nerve function and pinpoint the affected nerves.
