Before Your Visit - Things to Know

We believe that establishing a partnership before your spine care visit can achieve a better outcome for our patients. And we continue that partnership all the way through treatment and recovery. We understand that the idea of spine surgery can be overwhelming, and our team wants to ensure that your first visit runs as smoothly as possible. Therefore, we start the evaluation process during your initial phone call, so you can expect a doctor and physician assistant to be familiar with you even before your spine care visit. A team member will walk you through everything that will be required on your first visit such as the forms you will need to complete prior to your arrival, whether you will need to take new X-rays and which doctor you will be meeting with.

Forms to Fill Out Before Your Spine Care Visit

We know your time is valuable, therefore we prefer for you to complete and submit the forms linked below before your spine care visit.

What to expect when you arrive at our offices

We are located at 523 East 72nd Street, New York, NY 10021, about one block away from Hospital for Special Surgery, where we perform all surgical procedures.

Upon arrival for your spine care visit, you will enter through the main building lobby and proceed to the elevators. Our office is located on the 3rd floor. When you step out of the elevator, you will see a waiting area and an information desk where you will be asked to sign in and complete forms (if you did not do so online before your spine care visit). You will also receive a checklist detailing everyone you will meet during your visit (physician’s assistant, doctor, etc.) so you know who will be part of your care team.

After you have been checked in, a medical assistant will take you to an exam room. They will get your vitals as well as request your completed paperwork, imaging CDs/Films as well as additional medical records you may want to provide. Once this is completed, one of our physician assistants will meet you in the exam room and begin your appointment.

Directions and parking

For directions to our office and/or information about where to park when you arrive for your spine care visit, please click below. Please allow enough time prior to your appointment for parking as street parking is limited, and you will need to park in a garage. If you have someone accompanying you to the appointment, we recommend you be dropped off in front of the building.
